Bad breath is common, and it is fixable once you know where it comes from. About 85 to 90 percent of persistent cases start in the mouth, not the stomach. At Brampton Smiles, Dr. Azher Abbas works through a checklist to find the cause. Mouthwash only masks the smell for a few minutes, so we treat the source.
Common causes of bad breath
- Tongue coating. The rough back third of the tongue is the most common single source.
- Gum disease. Deep pockets trap bacteria.
- Dry mouth. From medications, mouth breathing, dehydration or some sleep devices.
- Decay or cracked teeth.
- Failing dental work. Leaking fillings, open crown edges or ill-fitting dentures.
- Tonsil stones.
- Diet and habits. Garlic, onion, coffee, alcohol, tobacco and very low-carb diets.
Morning breath or bad breath?
Saliva drops during sleep, so bacteria multiply overnight. Morning breath that clears after brushing is normal. If the smell comes back within an hour or two of brushing, or lasts all day, something is active and worth checking.
How we find the cause
- History: when it started, medications, health conditions, diet, tobacco and alcohol.
- Gum charting: pocket depths and bleeding at six sites per tooth.
- Tongue and soft tissue exam, including the tonsils, palate and throat.
- Saliva check for true dry mouth.
- Decay and dental work check, with X-rays and a close look at fillings, crowns, bridges and dentures.
Treatment depends on the cause
- Gum disease: deep cleaning (scaling and root planing), then maintenance every 3 to 4 months.
- Tongue coating: we show you the scraper technique, which reaches further back than most people expect.
- Dry mouth: saliva substitutes, a review of medications with your doctor, more water and xylitol products.
- Decay or failing dental work: repair or replacement.
- No mouth cause found: we refer you to your doctor or an ENT for sinus, tonsil, reflux or other causes.
How fast does treatment work?
Tongue coating often improves within days of proper scraping. Gum disease improves over 2 to 4 weeks as tissue heals. A failing filling or crown changes the picture right away once it is replaced.
Cost and coverage
An assessment is part of a regular exam. Treatment costs depend on the cause, such as a cleaning, deep cleaning or filling. You get a written estimate first. We direct-bill Sun Life, Manulife, Canada Life, Green Shield and CDCP, and offer 0% in-house payment plans. Evening appointments are available until 7 PM on Monday, Wednesday and Thursday.

Frequently Asked Questions
Does bad breath come from the stomach?
Rarely. The mouth is the usual source, so we check it first.
Why does my breath still smell after brushing?
Brushing mainly cleans the teeth. Bacteria also live on the tongue, along the gumline and between teeth.
Does mouthwash fix bad breath?
No, it only hides it for a short time. Alcohol-based rinses can dry your mouth and make things worse.
Can I check my own breath?
Self-checks are unreliable. A rough test is to scrape the back of your tongue with a spoon and smell it.
When should I book an appointment?
If it continues despite good home care, returns quickly after brushing, or comes with bleeding gums, a bad taste, dry mouth or a sore that does not heal.
Do you offer bad breath treatment in the evening?
Yes. Appointments run until 7 PM on Monday, Wednesday and Thursday.
